You will find your member number on the front of your Membership Card.

Information to be Engraved on Badge (Below)

Standard 2121 Members Badge - White Background w/ Blue Lettering - L1 Badge w/ 1 Emblem - #6 - Magnetic Mounting Clip on Reverse of Badge.

This is the Name you prefer to be called by: (Example: Your Full Name is "Robert Matthew Hastings", Name on Badge "Bob Hastings")
"Active Member" is the Standard Designation at the 2121 Lodge. Choose your Position or Choose "Other" and Type your Position in the Field Below.

Your replacement Badge will be processed with other badges and can sometimes take 3 to 4 weeks.

Once new Badges are Received, they will be placed on the "Secretaries Corkboard" outside the Secretaries Office for Pickup.

Badge "Replacement Cost" = $20.00 CASH ONLY

After submitting your order online, place a $20 dollar bill in an envelope and place it in the Secretaries Mailbox. Write your "Name and Replacement Badge" on the envelope.
